Login to the FORMS URL using the credentials that LineLeader provided you to access FORMS. ( If you do not recall this information, please reach out to LineLeader Support by emailing: support@lineleader.com)
To access completed Forms (for parents or guardians), please click on the “Completed” Tab (see screen shot below).
Please Note: It is NOT RECOMMENDED that a center/administrator edit parent completed FORMS, but instead to recommend parent to re-login and edit themselves.
